The Occupational English Test (OET) syllabus evaluates four core language skills: Listening, Reading, Writing, and Speaking. Designed for 12 healthcare professions, the exam measures English proficiency through real-life medical scenarios. The test lasts about 3 hours and 20 minutes and emphasizes clear communication, empathy, and accuracy in professional healthcare settings.

The table below summarizes the key highlights of the OET syllabus and exam format:

Feature

Details

Exam Name  Occupational English Test (OET)
Skills Tested  Listening, Reading, Writing, Speaking
Total Duration  About 3 hours 20 minutes
Exam Format  Computer-based or paper-based
Professions Covered  12 healthcare professions
Purpose  Assess English communication in healthcare settings
Score Range  0–500 (Grades A to E)

Section-Wise Breakdown of OET Syllabus

The OET exam syllabus is divided into four sections: Listening, Reading, Writing, and Speaking. Each section measures a candidate’s ability to communicate effectively in healthcare environments such as hospitals, clinics, and medical consultations.

Unlike general English tests, the OET syllabus focuses on real clinical communication, testing how well healthcare professionals understand patients, interpret medical documents, and communicate treatment plans clearly.

Below is a section-wise breakdown of the OET syllabus:

OET Listening Syllabus

The OET Listening section evaluates how accurately candidates understand spoken English in healthcare contexts.

Test recordings include patient consultations, professional discussions, and medical presentations.

The listening test contains three parts with 42 questions, lasting approximately 40 minutes

The listening tasks include:

Listening Part  Task Type  Key Skills Tested
Part A  Consultation extracts  Listening for specific information
Part B  Workplace conversations  Understanding purpose and detail
Part C  Presentations and interviews  Identifying opinions and attitudes
  • Part A includes two recorded consultations where candidates complete notes while listening to patient interactions.
  • Part B consists of short workplace extracts, such as briefings or handovers between healthcare staff. Candidates must identify the main purpose of the speaker’s message.
  • Part C features longer recordings such as interviews with healthcare professionals or medical presentations. These tasks test deeper listening comprehension and the ability to interpret opinions or viewpoints.

OET Reading Syllabus

The OET Reading section assesses how effectively candidates can understand healthcare-related texts.

The reading test usually lasts 60 minutes and consists of three parts with 42 questions

The reading tasks include:

Reading Part  Task Type  Skills Assessed
Part A  Expeditious reading  Skimming and scanning
Part B  Workplace texts  Understanding professional documents
Part C  Detailed reading  Analytical comprehension
  • Part A focuses on quick reading tasks where candidates must locate specific information from several short texts within a limited time.
  • Part B contains workplace communication texts such as emails, notices, or hospital guidelines. These tasks evaluate the ability to understand professional communication used in healthcare workplaces.
  • Part C involves longer texts such as journal articles or research-based discussions. Candidates must analyze the writer’s perspective, identify key arguments, and answer multiple-choice questions.

OET Writing Syllabus

The OET Writing section evaluates how clearly candidates can communicate information through professional healthcare documents.

Candidates receive case notes and must transform them into a formal document.

The writing tasks include:

Writing Task

Case-note based writing

Description  Convert patient case notes into formal communication
Output  Referral or discharge letter

The writing section lasts 45 minutes and requires candidates to produce one document based on the provided case notes.

Common document formats include:

  • Referral letters
  • Discharge summaries
  • Transfer letters
  • Advice letters

The writing assessment focuses on clarity, organization, professional tone, and appropriate use of medical terminology.

OET Speaking Syllabus

The OET Speaking section measures how effectively candidates interact with patients in healthcare scenarios.

The test is conducted through role-play exercises where candidates act as healthcare professionals.

Candidates participate in two role-plays, each lasting around five minutes, with preparation time before each task.

The speaking tasks include:

Task  Description  Skills Evaluated
Role Play 1  Patient consultation scenario  Explanation and empathy
Role Play 2  Healthcare communication task  Professional interaction

During these role plays, candidates may need to explain medical conditions, reassure patients, discuss treatment plans, or provide lifestyle advice.

Examiners assess both linguistic ability and clinical communication skills, ensuring candidates can interact clearly and professionally with patients.

Profession-Specific OET Syllabus

Although the OET exam format remains consistent, certain tasks vary depending on the healthcare profession of the candidate.

These variations ensure the test reflects real communication scenarios relevant to specific medical roles.

OET is currently available for multiple healthcare professionals. Below is the list of professions eligible for the OET exam:

  • Doctors
  • Nurses
  • Dentists
  • Pharmacists
  • Physiotherapists
  • Radiographers
  • Occupational therapists
  • Veterinary professionals
  • Dietitians
  • Podiatrists

OET Scoring System & Assessment Criteria

The OET scoring system measures performance on a numerical scale from 0 to 500, which corresponds to letter grades ranging from A to E

Most healthcare regulatory bodies require a minimum grade of B in each section to meet English language requirements for professional registration in countries such as UK, Australia, Ireland, and New Zealand.

Below is the score conversion scale used in the OET exam:

Score Range  Grade  Performance Level
450–500  Highly proficient communication
350–440  Strong professional English
300–340  C+  Moderate communication ability
200–290  Limited but adequate communication
100–190  Very limited English proficiency
0–90  Extremely limited English proficiency

Each section of the exam, Listening, Reading, Writing, and Speaking, is graded individually. Candidates receive a separate score and grade for every skill rather than a single combined overall score.

Healthcare regulators usually evaluate results based on minimum grade requirements in each section.

For example, many organizations require candidates to achieve at least grade B in all four sections to demonstrate sufficient English proficiency for professional practice.

Examiners assess candidates using several evaluation criteria, including:

  • Linguistic accuracy
  • Communication clarity
  • Organization of ideas
  • Professional tone

FAQs

Is the OET syllabus officially defined by the test provider or inferred from exam patterns?

The OET syllabus is broadly defined by the official Occupational English Test framework, which outlines the four skills assessed: listening, reading, writing, and speaking. However, detailed topic coverage is largely inferred from official sample tests and recurring question formats, as the exam focuses on practical healthcare communication scenarios rather than a fixed topic list.

Does the OET syllabus change every year or remain the same?

The OET syllabus remains relatively stable each year because it is based on real healthcare communication skills rather than academic topics. While the exam format and core skill areas stay consistent, minor updates may occur in task design or difficulty levels to reflect evolving professional communication standards in healthcare environments.

How is the OET syllabus different from IELTS or other English proficiency exams?

The OET syllabus focuses specifically on healthcare communication, unlike general English exams such as IELTS or TOEFL. Tasks are based on clinical scenarios, patient consultations, and professional documentation. This profession-focused approach evaluates how well healthcare professionals use English in medical settings rather than everyday or academic contexts.

Is the OET syllabus easier for healthcare professionals compared to general English exams?

Many healthcare professionals find the OET syllabus more relevant because it uses familiar medical contexts such as patient consultations and referral letters. However, the exam is not necessarily easier. It still requires strong English proficiency, clear communication skills, and the ability to understand professional healthcare discussions and documents accurately.

Which healthcare professions can take the OET exam?

The OET exam is designed for multiple healthcare professions, including doctors, nurses, dentists, pharmacists, physiotherapists, occupational therapists, and radiographers. Each profession receives role-play and writing tasks relevant to its clinical responsibilities, ensuring that the exam reflects realistic communication situations encountered in healthcare workplaces.

Are the listening and reading sections the same for all professions in OET?

Yes, the listening and reading sections of the OET exam are the same for all candidates, regardless of their healthcare profession. These sections assess general healthcare-related communication skills, such as understanding consultations, workplace conversations, and medical texts, which are considered essential across different healthcare fields.

How do the writing tasks differ between doctors, nurses, and other healthcare professionals?

In the OET writing section, candidates receive case notes relevant to their specific profession. Doctors may write referral or discharge letters to specialists, while nurses might prepare transfer or care update letters. The structure remains similar, but the clinical context and terminology align with the candidate’s professional role.

What types of medical scenarios are typically included in the OET exam?

The OET exam commonly includes scenarios such as patient consultations, explaining treatment plans, discussing symptoms, and writing referral letters. Listening tasks may feature hospital briefings or professional discussions, while reading passages often involve patient information leaflets, clinical guidelines, or healthcare-related research materials.

Is medical knowledge required to understand the OET syllabus?

The OET exam does not require advanced medical knowledge beyond a candidate’s professional training. However, familiarity with healthcare terminology and clinical communication is helpful. The test focuses primarily on language skills, ensuring candidates can understand and communicate medical information clearly in English-speaking healthcare environments.

What level of English proficiency is required to complete the OET syllabus successfully?

Most healthcare regulators require candidates to achieve at least a grade B in the OET exam, which indicates a strong level of professional English. This typically corresponds to advanced intermediate or upper-intermediate proficiency, where candidates can communicate complex medical information clearly and accurately with patients and colleagues.

Can candidates prepare for the OET syllabus without professional coaching?

Yes, many candidates prepare for the OET syllabus independently using official practice tests, preparation guides, and online resources. However, professional coaching can be beneficial for writing and speaking sections, where personalized feedback helps improve organization, grammar, and communication clarity required for achieving higher grades.

How long does it usually take to complete the OET syllabus preparation?

Preparation time for the OET syllabus varies depending on a candidate’s current English proficiency. On average, healthcare professionals spend about six to eight weeks preparing with regular practice. Candidates with strong English skills may require less time, while others may need additional weeks to strengthen writing and speaking abilities.

Are there official practice tests available that reflect the actual OET syllabus?

Yes, the official OET website provides authentic practice tests and sample materials that closely reflect the actual exam format. These resources help candidates understand question types, task instructions, and time management requirements, making them one of the most reliable ways to prepare for the OET exam.

How often can candidates retake the OET exam if they do not achieve the required score?

Candidates can retake the OET exam as many times as needed because there is no official limit on the number of attempts. However, it is recommended to review previous results, identify weaker sections, and improve targeted skills before scheduling another attempt to increase the chances of achieving the required grade.

Is it possible to improve only one section score without retaking the entire OET exam?

Yes, OET offers a feature called “OET on Computer with sub-tests,” allowing candidates in some regions to retake individual sections instead of the entire exam. This option helps candidates focus on improving a specific skill, such as writing or speaking, if they narrowly missed the required score.

How long are OET results valid for professional registration?

OET results are generally valid for two years from the test date, although the exact validity period may vary depending on the healthcare regulator or immigration authority. Candidates should check the specific requirements of the country or licensing body where they plan to work or study.
